Streamable HTTP

A transport mechanism for MCP that uses HTTP with streaming responses, designed for remote and enterprise deployments where servers may run on different machines or in containerized environments. Streamable HTTP requires the operator to handle authentication, TLS, and rate limiting — the protocol does not enforce these for you.
